Duties and Responsibilities:
· Assesses and determines health status of employees before and after illness in the absence of company physician, includes issuance of medical pass out slips (MPOS)
· Transports ill and injured employees to hospital in case of extreme emergency.
· Conducts health teaching to employees regarding medical/dental benefits, rules and regulations in connection to health.
· Maintains individual health records and keeps its confidentiality.
· Prepares and submits annual medical reports on medical cases, accident analysis, hospital cases to city health office and Dept. of Labor and Employment.
· Assists employees on their hospitalization needs and processing of pertinent papers for medical benefits /claims such as Philhealth, SSS and HMO.
· Coordinates with Health Maintenance Organization (HMO) for the company's health benefits.
· Prepares referral forms for pre-employment medical examination, follows up and monitors the results.
