The Medical City Ortigas is currently looking for SOCIAL WORKER:

Processes requirements of Divine Mercy patients; conducts interviews and explains the coverage of the application. Provides individual, family and group health counseling at TMC if needed. Assists in the preparation of Divine Mercy fund raising program.

RESPONSIBILITIES:

Handles Divine Mercy ProgramAssists in the preparation of Divine Mercy fund raising projects and donation.Conducts interview, explains the coverage and processes applications for Divine Mercy Program.Refers patients to DSWD for Social Case Study Report.Checks and validates Social Case Study Report, Certificate of Indigence and other supporting documents.Conducts ward visit to admitted patients of Divine Mercy.Handles Divine Mercy Out-Patient Clinic (OPD)Handles Liver Transplant PatientsRefers patients to other financial institutions, e.g. PCSO, DSWD, Senate, and or Congress, if neededProvides Social Work interventions to TMC patients if needed
